If you agree with the Terms of Engagement, go to the QDRO Order Form.

At the QDRO Order Form, choose the number of QDROs and select your unique user name and password. (The HELP function will assist you in identifying your type of plan.) Make sure you print out or make a note of your user name and password so that, if you choose, you can save your work at any time and come back later to complete the forms. If you forget your password, contact us.

Once you make your selection on the QDRO Order Form, proceed to payment. We accept VISA and Mastercard on-line. The site is secured for your protection.
After your payment has been accepted, you will be given a set of forms specific to your order. Complete the forms on-line.

When the forms are complete, submit them to us on-line. If you order more than one QDRO, you may submit the forms for each QDRO separately, as you complete them. You do not have to wait and submit the forms for the entire order all at once. We prepare each QDRO after submission of the completed forms for that QDRO - we do not have to wait until you have submitted all the forms for your entire order.

You also must mail or fax (1) the first page of your Judgment of Dissolution or Marital Settlement Agreement that states the name of the court and case number, and (2) the pages of the Judgment or Marital Settlement Agreement detailing the ordered division of the plan to:
